This video shows how to export Schematic and PCB Layout files to Altium Designer using P-CAD ASCII format.
Firstly, export the Schematic design:
Then, export the PCB Layout design:
Finally, import these files via "Import Wizard", following the instructions from Altium website:

I followed the video, but when my schematic loaded in Altium, some nets were not connected. Like for example, I had a net named "VREF" that is connected to three different ICs but after importing to Altium, some of these nets are removed and left with net names like "NetIC_8". Any suggestions on how to fix this issue?
