From help:
Pin Terminal – establish pin terminal shape (obround, rectangle, polygon, D-shape); you can add and use up to 4 shapes to get the desired pin projection. Enter values to define shape size, X and Y offset relative to the pad shape and rotation angle.
In pattern editor 4.0.0.2 place an smd pad. Now trying to extend the shape of the pattern: Hover over pad, Pin terminal, adding two rectangles.
But this does not change the shape of the pad, see attachament.

As far as I can tell, the Pin Terminal is used to create a drawing that is supposed to represent a component pin outline and is not meant to be part of a pad. If I'm guessing correctly here, the pin terminal can be used by the DipTrace package generator to create the pin of a basic 3D model. Could be wrong, of course.
